One of the possible causes of premature birth is cervical insufficiency: the cervix shortens and the cervix dilates prematurely. It most commonly occurs between the fourth and sixth months of pregnancy and can have various causes. Sometimes it is a consequence of previous difficult births or a previous miscarriage; infections or uterine surgery can also lead to cervical insufficiency.

In the sense of 'bloodless cerclage', the cerclage- & A support pessary for cervical insufficiency relieves the pressure directed on the cervix.

Mechanism of action:
- the cerclage & A support pessary closes the cervical canal like a band ('bloodless cerclage').
- An offset central hole in the pessary receives the cervix and pushes it backwards (sacralization of the cervix)
- The cervix is relieved by the pessary being broadly supported on the pelvic floor.
